| I understand that I must be
present for at least 30 minutes of the duration of the workshop if my child
is under 8 years old. Consent for Photography of Children, Data Storage and Image Manipulation / Transmission of Data via the Internet to The Fairy School or high school roadshow website and to Commercial Printers. I/we the parents/legal guardian of the above named child consent to the photography of the above child by one of The Enchanted photos photographers. We consent to the storage of these images, both pre and post transformation, on the computer of the Fantasy Photos, or on any other data storage medium belonging to, and retained by, the photographer for archiving purposes as well as the transmission of images to ourselves for the purpose of choosing the image to be transformed. We consent to the photographer transmitting the chosen image via the Internet for transformation into Fantasy Images that will be created by Enchanted Photos, or their employees. We consent to the photographer transmitting the completed images via the internet to commercial printers of his choice to fulfil orders placed. Signed Date . |
